Job summary

Fairview Park Hospital in Dublin, GA seeks an RN Case Manager (PRN) to support patients and families through all care stages. You will coordinate services, develop individualized care plans, and ensure safe transitions by collaborating with physicians, nurses, and the care team.

The role requires RN licensure, ADN or higher, 1–3 years in clinical/hospital nursing, and preferred Case Management certification. Join a facility recognized for quality and patient-centered care.

Qualifications

• Must be a graduate from an accredited school of Nursing (ADN or higher). BSN preferred.
• Licensed as a Registered Nurse in the practicing state or active compact license.
• 1-3 years of clinical, hospital nursing experience required.
• Prior case management experience required for PRN role.
• Certification in Case Management or Utilization Review preferred.

Responsibilities

• Guide patients and families through orientation, discharge criteria, and rights and explain insurance benefits.
• Coordinate education for patients, families, and caregivers for treatment planning and conferences.
• Develop individualized treatment plans reflecting patient strengths and goals.
• Complete psychosocial assessments and discharge planning evaluations with clear documentation.
• Collaborate with Rehab Program Director, Facility Case Management Director, and care team to ensure seamless care.
• Promote HCA Healthcare values in every patient and family interaction.
