Qualifications

• Active nursing license in Arizona.
• At least 2 years of nursing experience, with 1 year in home health or hospice.
• Strong knowledge of clinical nursing practices, including assessments and care plans.
• Excellent communication with patients, families, and multidisciplinary teams.
• Ability to work independently and manage multiple patients and tasks.
• Valid driver’s license and reliable transportation for home visits.
• Compassionate, detail-oriented, and committed to patient care.

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans with physicians.
• Administer medications, wound care, and other nursing treatments at home.
• Monitor patient progress and adjust care plans to support recovery and comfort.
• Educate patients and families on managing medical conditions, medications, and home care techniques.
• Provide health assessments, coordinate care, and communicate with the patient’s care team.
• Ensure patient safety and implement infection control and healthcare practices.
• Document all patient visits, treatments, and progress per policies.
• Advocate for patient needs and ensure continuity of care through interdisciplinary collaboration.
• Offer emotional support and guidance to patients and families.
