RN Case Manager - PRN

What You Must Have:

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State Registered Nurse License
  • Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
  • Two years of experience as an RN, preferably in a healthcare/clinical setting

What You Will Do:

  • Conduct initial and ongoing comprehensive assessments of the impact of the terminal disease on patients' physical, psychosocial, and environmental needs, including risk for grief, cultural and spiritual considerations, verbal and non-verbal communication
  • Implement the individualized Plan of Care (POC) and recommend revisions as necessary
  • Collaborate on care among patients, caregivers, physicians, and healthcare professionals
  • Attend Inter-Disciplinary Group (IDG) meetings, coordinate and participate in patient care planning, prioritize needs, and promote care coordination
  • Perform on-call responsibilities and provide on-call services to patients and families as assigned
  • Ensure timely submission of all documentation
  • Perform other duties as required
  • Ensure overall compliance with local, state, and federal laws, Medicare regulations, and personnel policies and procedures
