Suncoast Hospice, a member of Empath Health, is seeking a bilingual (PRN) RN Case Manager to join the Empath team.

Coverage area is Hillsborough County.

Responsibilities

1. Identify and coordinate patient and family needs, providing preventative and supportive care to terminally ill patients and their families.
2. Utilize assessment, intervention, and teaching skills to maximize patient comfort and improve quality of life for patients and their families.

Position Requirements

• Education and/or Experience: Associate’s degree (A.A.) or equivalent from a two-year college or technical school; minimum of one year related experience and/or training; or an equivalent combination of education and experience.
• Licensure:
• Valid Florida license as a Registered Nurse
• Valid Florida State Driver’s License
• Bilingual - fluent in English and Spanish
• Prefer one (1) year of experience in an acute or post-acute care setting
