Job summary

UAB Medicine in Birmingham, AL is seeking a Registered Nurse for Case Management on a PRN days schedule. The role coordinates the interdisciplinary plan of care from admission to discharge and collaborates with patients, families, and the care team to ensure successful transitions.

The ideal candidate holds an Alabama RN license (or eligibility), a Diploma in Nursing, and a BLS certification, with at least one year of RN experience.

Qualifications

• BLS certification within 30 days of hire date.
• Alabama RN license or eligibility required.
• Diploma from an accredited nursing program and professional licensure at time of hire.
• Minimum 1 year RN experience.

Responsibilities

• Coordinates the interdisciplinary plan of care from admission to discharge.
• Creates care plans across the continuum incorporating patient/family preferences and values.
• Monitors medical necessity and level of care via assessments and records.
• Advocates for resources and removal of barriers.
• Maintains dialogue with supervisor and care transition team for effective implementation and reevaluation of health plan.
• Serves as resource for medical record documentation and post-discharge care options.
