Qualifications

• RN license in Alabama required.
• BSN within 36 months of placement required.
• 3 years of experience in chronic disease management, case management, utilization management, or acute clinical care required.
• Maintains RN license in Alabama and current BLS certification.

Responsabilités

• Assesses patients within assigned population, including history, treatments, needs, and anticipated services.
• Assigns working DRG and monitors LOS per medical necessity and Interqual guidelines.
• Develops relationships with patient/family and care team to move through continuum of care efficiently.
• Prioritizes activities in high risk, high cost, and problem-prone areas.
• Acts as advocate for patient health care needs and coordinates with payors as needed.
• Performs admission/concurrent utilization review in line with contracts and review policies.
