Qualifications

• RN license required.
• Bachelor’s degree or an associate degree in nursing (ADN/BSN) accepted.

Responsibilities

• Guide patients and families through care journey and discharge planning.
• Coordinate education for patients and families on treatment plans.
• Develop individualized treatment plans reflecting patient needs and goals.
• Complete psychosocial assessments and communicate needs to the care team.
• Collaborate with Rehab Program Director and interdisciplinary team for patient-centered care.
• Promote Wesley Healthcare values in all patient interactions.
