Job summary

Lighthouse Hospice, Inc. is seeking a Per Diem Hospice RN Case Manager to join our compassionate nursing team serving Mercer, Burlington, and Camden counties.

You will deliver hands-on hospice care, coordinate services, and guide patients and families through their hospice journey. As an RN Case Manager, you will educate and advocate for patients, collaborate with the Clinical Director, and participate in on-call duties.

Qualifications

  • Hold current unencumbered RN license.
  • Minimum of 1 year RN experience in medical-surgical/acute care; Hospice experience a plus.
  • Must be proficient with computers and EMR.
  • Current CPR certification if required by state.
  • Valid driver’s license, vehicle insurance, and reliable transportation.

Responsibilities

  • Respond to clinical referrals with care and expertise.
  • Collaborate with Clinical Director to assess client eligibility for hospice services.
  • Provide direct nursing care and support to ensure comfort and quality of life.
  • Deliver nursing care rooted in evidence-based practice.
  • Educate patients and families to participate in personalized care plans.
  • Participate in on-call duties as part of the team.

Lighthouse Hospice is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community.

Schedule: Per Diem

Territory: Mercer, Burlington and Camden County

In this rewarding role, you’ll deliver hands‑on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. Responsibilities

Qualifications
  • Hold current unencumbered license as a Registered Nurse.
  • Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
  • Must be computer proficient in typing and various programs, including background in EMR.
  • Possess and maintains current CPR certification if required by state.
  • Current driver’s license, vehicle insurance, and access to dependable transportation.

$41.00 - $46.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
