Job summary

UCLA Health in Los Angeles is seeking a nurse to join our team in a per diem role with potential to convert to a career. The position is fully on-site with weekend rotation and 10-hour day shifts, 7:00am-5:30pm, including holidays.

Using your advanced practice nursing skills, you will assess and coordinate care for a diverse patient population, collaborate with a multidisciplinary team, develop individualized care plans using evidence-based tools, and perform utilization review to ensure safe,

Qualifications

• BSN or MSN degree required (ASN accepted for current UCLA Health Nursing staff).
• California RN license and BLS certification.
• Recent experience in case management, utilization management and discharge planning.
• Minimum of three years of acute hospital experience; or the equivalent of education and experience.
• Strong leadership abilities.
• Systems planning and patient care management in high-volume environments.
• Excellent communication, interpersonal, organizational and analytical skills.
• Ability to work effectively and collaboratively with interdisciplinary teams.
• Knowledge of a large university teaching hospital setting.

Responsibilities

• Assess and coordinate care for a diverse group of patients.
• Collaborate with a multi-disciplinary health care team and patients' families to ensure safe and effective care.
• Develop and implement individualized care plans using evidence-based tools for risk stratification.
• Perform utilization review and ensure delivery of concurrent and post-hospital care.
• Help drive performance improvement efforts.
