Job summary

Cleveland Clinic Weston Hospital is seeking a Per Diem RN Case Manager to coordinate transitional care and work closely with interprofessional teams to promote patient advocacy and holistic care.

You will assess risks, develop individualized care plans, and ensure smooth handoffs to next level of care or community. BSN preferred; minimum RN experience required with utilization review/quality/care management preferred.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• BSN preferred.
• Three years of recent RN clinical experience.
• Two years of recent experience in utilization review, quality, or care management preferred.

• Complete effective handoffs to the next level of care or community.

Requirements

Education:

• Graduate from an accredited school of Professional Nursing
• Bachelor’s of Science in Nursing (BSN) preferred

Experience:

• Three years of recent RN clinical experience
• Two years of recent experience in utilization review, quality, or care management preferred

Licenses & Certifications:

• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification required
• Certification by an approved entity within one year of eligibility preferred (CTM, ACM, CCM, or AHA)
