Position Purpose

At Saint Francis Hospital, our RN Case Manager is responsible for coordinating patient care, assessing needs, and developing care plans. They collaborate with healthcare providers, monitor patient progress, and ensure smooth transitions between stages of care while advocating for patients' well-being.

What You Will Do

• Serve as a driver of a collaborative process that coordinates, monitors, and plans healthcare services throughout the patient's illness.

Minimum Qualifications

• Education: Bachelor of Science in Nursing (BSN) degree
• Licensure: Current RN licensure in Connecticut
• Experience: Five years of diversified, progressive experience in hospital case management, acute care, or related settings, with understanding of medical utilization criteria, HCFA/OBRA, Connecticut regulatory issues, and JCAHO accreditation.
• Certification: BLS certification required
