A leading healthcare facility is seeking a Registered Nurse to join their care team at Wentworth-Douglass Hospital. The role involves coordinating healthcare services, managing referrals, and engaging with patients and their families to develop comprehensive care plans. Ideal candidates will have an Associate's Degree in Nursing, relevant RN license, and strong communication skills. Experience in case management and proficiency with Microsoft Office is preferred.

Serves as a core member of the care team, coordinates and supports healthcare within the facility, and manages referrals for services outside the clinic. Provides outreach and enrollment services to meet program eligibility requirements and supports community and family planning. Focuses on developing and coordinating community service plans, partnering with service providers, families, and patients to create healthcare plans and identify additional services.
Education
Associate's Degree in Nursing required; Bachelor's Degree preferred. Can this role accept experience in lieu of a degree? No.
