Qualifications

• Active, unrestricted Registered Nurse (RN) license in the State of Florida (must be maintained).
• 3+ years of clinical experience.
• 1+ year of case management experience preferred.

Responsibilities

• Coordinate care across providers and service settings.
• Advocate for patients and families to ensure access to necessary resources.
