Qualifications

• Active, unrestricted RN license in Georgia.
• 3+ years of clinical experience.
• 1+ year of case management experience preferred.

Responsibilities

• Coordinate care across providers and service settings.
• Advocate for patients and families to ensure access to resources.
• Provide education to patients, caregivers, and support systems.
• Monitor patient progress and adjust care plans as needed.
