Job summary

HCA Florida Fort Walton-Destin Hospital is seeking an RN Case Manager to support patients and families through the care journey, coordinating services and developing individualized care plans. You will work with physicians, nurses, and leaders to ensure safe transitions and consistent quality across the continuum of care.

The role focuses on guiding patients, educating families, and coordinating resources to promote patient-centered care and optimal outcomes in a fast-paced hospital setting.

Qualifications

• Active RN license in Florida
• Associate degree or nursing certificate required
• Ability to coordinate care and communicate with patients and families

Responsibilities

• Guide patients and families through orientation, discharge planning and education
• Coordinate education for patients, families, and caregivers to support treatment planning and goals
• Develop and implement individualized treatment plans reflecting patient needs and recovery goals
• Complete psychosocial assessments and communicate needs across the care team
• Collaborate with Rehab Program Director and interdisciplinary team to ensure seamless, patient-centered care
• Promote HCA Healthcare values through every patient interaction
