Job summary

St. Croix Hospice is seeking a compassionate Registered Nurse Case Manager to provide individualized, holistic support to patients in their homes. You will collaborate with a care team to develop personalized plans of care, lead meaningful end-of-life conversations, and ensure symptom management with empathy and clinical excellence.

Qualifications

• Associates, Bachelors, or Masters degree in an accredited nursing program.
• 1+ years of recent acute care experience; hospice/home health preferred.
• Ability to complete comprehensive patient and family assessments.
• Develop and update individualized care plans with patient and family.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and manage care transitions.

Responsibilities

• Deliver holistic care to patients in their homes as part of the care team.
• Lead end-of-life conversations with patients and families with empathy.
• Coordinate services to ensure symptom management and comfort.
• Document and communicate care plans; adjust as needs evolve.
• Ensure reliable transportation and on-call availability as required.
