Qualifications

Job description

RN Case Manager

What You Must Have:

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State Registered Nurse License
  • Current Driver's License, reliable transportation, and ongoing valid auto liability insurance
  • Two years of experience as an RN, preferably in a healthcare/clinical setting

What You Will Do:

  • Conduct initial and ongoing comprehensive assessments of patients' physical, psychosocial, environmental needs, and ADLs, including risk for grief, cultural and spiritual considerations, verbal and non-verbal cues
  • Implement individualized Plans of Care (POC) and recommend revisions as necessary
  • Collaborate with patients, caregivers, physicians, and healthcare professionals to coordinate care
  • Attend Inter-Disciplinary Group (IDG) meetings, coordinate and participate in patient care planning, prioritize needs, and promote care coordination
  • Perform on-call responsibilities and provide on-call services to patients and families as assigned
  • Ensure timely submission of all documentation
  • Perform other duties as required
  • Ensure compliance with local, state, and federal laws, Medicare regulations, and organizational policies
