Job summary

Qualifications

• Current CA RN license.
• Current BLS provider card (Case Management certification preferred).
• Diploma, AA, or BS in Nursing; MS in Nursing preferred.

Responsibilities

• Coordinate care across the continuum and manage patient throughput in partnership with a multidisciplinary team.
• Oversee care coordination and post-hospital needs; ensure timely, efficient use of services.
• Refer patients to clinical social workers for psycho-social issues and complex discharge planning.
• Collaborate with insurers, medical groups, and community resources to optimize care and resource use.
