Job summary

Henry Mayo Newhall Hospital in Santa Clarita, CA is seeking an experienced RN Case Manager to coordinate care across the continuum, perform utilization review, and connect patients, families, and nursing units with social work and post-hospital planning.

The role requires CA RN license, BLS, and case management certification preferred; two years in acute care or five years nursing experience preferred. Shift is Full Time 8:00AM-4:30PM with a pay range of $51.31 to $78.76 per hour.

Qualifications

• Must hold a current CA RN license.
• Current BLS provider card is required.
• Case Management certification preferred.

Responsibilities

• Coordinate care across the continuum of care and oversee post-hospital planning.
• Perform utilization review and act as a resource to patients, families, and units.
• Interface with insurance companies, medical groups, and community resources to ensure appropriate service use.
• Work as part of a multidisciplinary team to manage patient throughput during acute episodes.
