Job Title: RN Case Manager

Reports To: Waiver Program Supervisor

Scheduled Hours: Full-Time (37.5 hrs. per week)

FLSA Status: Exempt (Salary)

Job Description

The RN Case Manager provides direct care management and continuity of services to clients through assessing, planning, linking, monitoring, and advocating.

Essential Functions

• Conducts comprehensive participant assessment to determine financial eligibility, identify care needs, recommend services.
• Works closely with Waiver team SW as part of the care management team including the development of participant care plans, including determination of frequency and duration of social services required under the care plan.
• Assessment and planning are carried out using a team approach consisting of a RN and a SW as mandated by the MI CHOICE Waiver MDHHS Contract in order to provide a holistic, person-centered perspective. This also provides a foundation for thorough planning of meeting medical needs as well as environmental and psycho-social concerns.
• Links, coordinates, and monitors services through a network of service providers.
• Documents in an accurate and timely manner all assessments, service planning, and service/client progress to ensure effective communication with colleagues and to comply with agency and MDHHS policy and procedure record keeping requirements.
• Provides to the Waiver Supervisor all required and requested reports to ensure timely service delivery feedback, data tracking, and efficient use of caseload management.
• Establish and advocate as an effective liaison between the MI Choice Waiver Program and outside providers.
• Participate in case reviews and consultations.
• Participates in Quality Assurance case reviews, follow up and procedures.
• Monitor treatment plan follow-up, services to participant and over-all conditions. Works with the providers to educate, collaborate and increase their awareness of the client’s long-term care needs and make necessary improvements as needed. Follow-up on problem areas as appropriate.
• Participates in all team meetings, gives, and receives feedback.
• Other duties as assigned.
  
  
  

Qualifications

• The position requires Registered Nursing licensure, with a BSN preferred.
• Must have licensure with the State of Michigan as an RN and maintain licensure.
• At least one year of relevant work experience preferably with the elderly and in- home service delivery.
