Job summary

TruHealth in Mount Sterling, KY is seeking an RN Case Manager to manage and support care coordination for Medicare Advantage members. You will visit patients in person or by telehealth, correct errors with physicians, and collaborate with the health care team to assess, plan, implement and evaluate patient care plans.

The role requires a self-starter who can handle multiple priorities, work with minimal supervision, and communicate effectively with multiple teams.

Qualifications

• Minimum of 2 years of clinical nursing or geriatric rehabilitation experience.
• 2 years managed care experience required.
• 3–5 years case management experience in a managed care environment preferred.

Responsibilities

• Complete Health Risk Assessments for members as assigned.
• Initiate, update and/or revise care plans as needed.
• Maintain a caseload of patients as assigned.
• Coordinate outpatient discharge planning based on patient needs, clinical circumstances and benefit coverage.
• Participate in Managed Care audits and generate reports/documents.
• Educate members and Home/Facility staff on health options and learning needs.
• Respond to day-to-day problems in care coordination and communicate with facilities.
