Job summary

OhioHealth is seeking a Care Manager to collaborate with patients, families, and providers to deliver organized, cost-effective, patient-centered care. The role emphasizes disease prevention, management strategies, and coordination across the care continuum within the Unified Medical Center network.

The position requires an RN from an accredited nursing program, an active Ohio license (RN/APRN), and BLS certification. 40-hour work week in a hospital setting at UM Care Coord-North Hub.

Qualifications

• RN from an accredited school of Nursing with a current unencumbered RN or APP-RN licensure in the State of Ohio.
• Bachelor's degree in nursing required; Associates hired after 1/1/2020 must have their Bachelor’s degree in nursing.
• BLS certification issued by American Heart Association.

Job Description Summary

Evidence-based interventions are applied to populations for cost efficient and outcome-based care. Outcomes include disease-based quality measures, patient satisfaction metrics, access to services, and hospital/ED utilization rates.

Additional Job Description

RN from an accredited school of Nursing with a current unencumbered RN or APP-RN licensure in the State of Ohio. Bachelor's degree and 2-5 years recent nursing practice experience with knowledge of acute hospital, community or public health nursing concepts and resources. Computer skill and data entry. Knowledge and skill in clinical assessment and disease management.

BLS issued by American Heart Association.

Associates hired after 1/1/2020 must have their Bachelor’s degree in nursing .
