AltaMed Health Services in California seeks an experienced RN case manager to lead the care coordination team for PACE participants in ambulatory settings. The role emphasizes coordinating care, improving outcomes, and guiding clinical pathways within a community health framework.

The candidate must hold a CA RN license, have at least 1 year of geriatric nursing experience, and be familiar with managed care/case management in ambulatory or inpatient settings. BLS certification is required.

Qualifications

• Current valid License as a Registered Nurse through the California Board of Registered Nursing.
• A minimum of 1 year of experience in Geriatric Nursing is preferred.
• A minimum of 1 year of experience in managed care/utilization or case management in ambulatory or inpatient setting.

Responsibilities

• The nurse (RN) case manager/care coordination team leader has the responsibility to manage all PACE participants in the ambulatory setting.
