Job summary

Cedars-Sinai is seeking a Registered Nurse Case Manager to plan and coordinate care from pre-hospitalization through discharge. You will work with the healthcare team to ensure collaborative care, review treatment against criteria, and arrange home care and discharge planning with social workers.

You will educate patients and families on hospitalization and ongoing care, and complete required case management reports for regulatory bodies and insurers.

Qualifications

• Associate's degree in Nursing required.
• Bachelor's degree in Nursing preferred.

Responsibilities

• Performs evaluation and assessment within established timeframe.
• Documents appropriate reviews for assigned patients using utilization review tools.
• Provides telephonic review for identified contracted/private patients in coordination with reviewers.
• Keeps patients informed of progress and information related to disease progression.
• Coordinates discharge planning with social workers and arranges home care equipment and needs.
• Educates patients and families on hospitalization and continuing care.
• Completes case management reports for regulatory bodies, health plans, and insurers.
• Interacts professionally with patient/family and involves them in care planning and discharge needs.
