Cedars-Sinai is seeking a Case Manager Care Coordinator to oversee patient care from admission to discharge, assessing needs and coordinating services for safe transitions to next levels of care.

The role requires nursing licensure (RN) with 2+ years in acute care or 1 year in acute care plus 2+ years in case management, plus BLS and CCM desirability. Strong teamwork and resource utilization are essential.

Formación

• Bachelor's Degree Nursing - preferred; Assoc. Degree/College Diploma Nursing - minimum.
• 2 years In acute care nursing - Medical Surgical/Tele/ICU/LTACH or 1 year in acute care nursing (Medical Surgical/Tele/ICU) with 2 or more year of care management experience (UM or DC Planning). - minimum.
• RN State License - Upon Hire minimum Acute nursing experience
• Basic Life Support (BLS) - Upon Hire minimum BLS Required
• Certified Case Manager - 2 years preferred Experience and certification in case management preferred

Responsabilidades

• Coordinate care from admission through discharge.
• Collaborate with inter-disciplinary teams to build a case management plan.
• Ensure patient needs (clinical, psychosocial, financial) are addressed in transition of care.
