Job summary

Paycom - ATS is seeking an experienced RN Case Manager to coordinate patient care across disciplines and oversee visit frequency for episodes. You will communicate with physicians, perform OASIS data collection, and ensure timely reporting of lab values.

Role requires an active RN or LPN license, CPR certification, and strong communication and critical thinking skills. You will document assessments and collaborate in weekly case conferences and monthly meetings.

Qualifications

• Must hold a current, unencumbered RN or LPN license.
• Certification in CPR is required.

Responsibilities

• Provide patient care with respect for patient rights and dignity; coordinate visits and episode care.
• Serve as the main contact for all disciplines involved in patient care and communicate with physicians and offices.
• Perform accurate OASIS data collection and ensure medication profiles are current; report lab values timely.
• Attend weekly case conferences and monthly case management meetings; document patient history and assessments.
