Job summary

Hines & Associates Inc is seeking a Nurse Case Manager to translate clinical nursing experience into patient-centered care plans, advocate for patients, and coordinate services. This role supports Oncology, Chronic Condition, and Behavioral Health tracks.

The position is full-time with a simple Monday–Friday schedule, in-office initially and with a hybrid option after orientation. It offers competitive benefits and 401K contributions, with generous time-off policy.

Qualifications

• RN license in state or reciprocation state
• Successful completion of case management orientation
• Minimum 2 years full-time acute inpatient setting or specialty area (ICU/ER/Oncology/NICU/Acute Rehab)
• Excellent communication skills
• Basic typing/computer knowledge with 35 WPM

Responsibilities

• Develop a plan for meeting health care needs of the patient
• Be the patient advocate and coordinate care
• Utilize clinical nursing experience in case management
• Possibly specialize in Oncology, Chronic Condition, or Behavioral Health
