Job summary

Health Care for the Homeless in Baltimore seeks an RN Case Manager to provide medically related case management for housing services clients, coordinating with medical providers and the multidisciplinary team. Responsibilities include nursing assessments, home visits, health education, documentation in EHR, and integration of Harm Reduction and Housing First principles to improve housing outcomes.

Applicants must be Maryland-licensed RNs with a BSN, valid MD driver’s license, and 2 years of case

Qualifications

• Must hold Maryland RN license and BSN.
• Two years of case management and two years of clinical nursing experience.
• Experience with behavioral health preferred.

Responsibilities

• Provide nurse case management and care coordination for Housing Services clients.
• Perform nursing assessments and administer medications.
• Conduct home and community visits and document in EHR.
• Support integrated treatment planning with SMART goals and trauma-informed care.
