Moments Hospice in Wisconsin is seeking an RN Case Manager to deliver compassionate, patient-centered home healthcare on the road.

You will conduct initial assessments, refine care plans, provide hands-on nursing, educate patients and families, and coordinate with the healthcare team to ensure comfort during end-of-life care. A valid RN license and reliable vehicle are required.

Qualifications

• Licensed RN in state you work in.
• Proficiency with EMR software.
• Excellent planning, organizing and communication skills.
• Ability to solve problems autonomously.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
