Job summary

Bright Heart Hospice is seeking a dedicated Registered Nurse Case Manager to coordinate patient care across the healthcare journey. You will collaborate with patients, families, and interdisciplinary teams to ensure access to services, education, and support that promote quality of life.

The ideal candidate holds an Oklahoma RN license, has strong communication and organizational skills, and can manage multiple cases independently while maintaining HIPAA compliance.

Qualifications

• Current RN license in Oklahoma.
• Experience in case management preferred but not required.
• Strong interpersonal and communication skills.
• Knowledge of HIPAA regulations and patient privacy.
• Ability to work independently and manage multiple cases.
• Excellent organizational skills and attention to detail.

Responsibilities

• Assess patient needs and develop individualized, comprehensive care plans.
• Coordinate care with multidisciplinary healthcare teams to ensure continuity of services across various care settings.
• Monitor patient progress and update care plans based on ongoing assessments and changing needs.
• Educate patients and families regarding health management, treatment options, and available community resources.
• Advocate for patients by facilitating access to appropriate healthcare services and support systems.
• Maintain accurate and timely documentation in accordance with organizational policies and professional standards.
• Ensure compliance with HIPAA regulations and maintain strict patient confidentiality.
• Collaborate with providers and specialists in areas including acute care, managed care, ICU, PICU, trauma, geriatrics, and other specialties as needed.
• Provide compassionate nursing support while promoting a safe and supportive environment for patients and families.
