Ardencares seeks a dedicated Registered Nurse to join our hospice team as a Case Manager. You will coordinate patient care and oversee services to ensure comfort, dignity, and quality of life during end-of-life care.

You will collaborate with patients, families, and an interdisciplinary team to implement personalized care plans that address individual needs, preferences, and goals within our compassionate hospice program.

Qualifications

• Licensed RN with ability to coordinate patient care.
• Hospice or end-of-life care experience preferred.
• Strong communication with patients, families, and team members.
