Job summary

Amedisys is seeking an RN Case Manager to join our respected home health and hospice team in the United States. You will perform comprehensive patient assessments, collaborate with the care team to develop plans of care, and coordinate referrals as needed to ensure the highest standard of home-based care.

The role emphasizes documentation accuracy, ongoing patient and family education, and close communication with the clinical manager.

Qualifications

• One year of nursing experience.
• Current RN license for the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines or documents rationale for not doing so.
• Maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes and outcomes.
• Educates patients, families, and caregivers on care techniques.
• Communicates regularly with the clinical manager and care team.
• Supports on-call responsibilities for patients and families.

• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
