Job summary

Optum, part of the UnitedHealth Group family, is seeking an RN Case Manager to coordinate compassionate hospice care in the Bourne/Falmouth areas with orientation at the Hyannis care center.

The role requires an active RN license, CPR, and reliable transportation. A sign-on bonus of $10,000 is offered for external candidates, with annual salary range $87,560–$131,340 based on full-time employment.

Qualifications

• Current RN license valid in state(s) assigned.
• 1+ years of nursing experience.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

As an RN Case Manager, you'll coordinate compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient's needs.
