Job summary

Optum is seeking a Registered Nurse Case Manager to oversee the plan of care for hospice patients, coordinating with the care team to support physical, psychosocial, emotional, and spiritual well-being.

You will evaluate performance, develop and implement treatment plans, and work with families facing end-of-life needs in Louisiana, with a focus on high-quality, compassionate care.

Qualifications

  • Current and unrestricted RN licensure in the state of practice.
  • Valid driver’s license, vehicle insurance, and access to dependable transportation or public transit.

Responsibilities

  • Perform initial nursing evaluation within 48 hours of referral to determine hospice eligibility.
  • Identify patient/family needs and reassess at least every 15 days; initiate and coordinate the plan of care.
  • Document problems, goals, interventions, and responses; collaborate with IDG.
  • Instruct and supervise patient/family in self-care when appropriate.

As the Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered. You’ll also develop the patient’s Plan of care in collaboration with other professionals and implement treatment strategies based on scientific nursing theory that promotes physical, psychosocial, emotional, and spiritual well‑being. Familiar with the concepts and needs of patients/families who are facing death and dying.

Primary Responsibilities
  • Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty‑eight (48) hours of referral
  • Identifies the patient’s/family’s physical, psychosocial, and environmental needs and reassesses as needed, no less than every fifteen (15) days
  • Initiates and coordinates the plan of care
  • Documents problems, appropriate goals, interventions, and patient/family response to hospice care
  • Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily
  • Instructs and supervises the patient/family in self‑care techniques when appropriate

Maintains accurate and relevant clinical notes regarding the patient’s condition

  • Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualifications

Current and unrestricted RN licensure in the state of practice

Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation

  • 2+ years of full‑time experience working as an RN
    • However, two years of full‑time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse
Preferred Qualifications
  • 1+ years of clinical experience
  • Current CPR certification or ability to complete within 90 days of hire

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $75,620 to $113,430 annually based on full‑time employment. We comply with all minimum wage laws as applicable.
