In collaboration with the multi-disciplinary Reid Hospice team, the Hospice Case Manager RN is responsible for coordination of all care for their assigned hospice patients; this includes admission, discharge, re-certification, and planning of care. The Case Manager RN will work closely with the Hospice Interdisciplinary Group (IDG), including patients and families, as well as Reid Health Leadership, Medical Staff, and physician’s practice office personnel; will work with representatives of the community, federal, and state agencies, as well as with outside review and accreditation agencies.

Overview of Responsibilities

The following list describes the essential duties of this role. Individuals in this role may not perform all of these duties, or may perform additional, related duties not listed here.

  • Accepts patient assignments by Hospice Charge Nurse or Triage Nurse.
  • Uses assessment skills and interviewing techniques to identify problems and patient/family physical, psychosocial, spiritual, and educational needs.
  • Guides, supports, and provides education to hospice patients and families.
  • Develops and modifies the care plan according to the patient’s needs, expected outcomes to be achieved, and coordinates all ancillary services.
  • Carries out physician’s orders in compliance with policies and procedures.
  • Informs the physician(s) and other personnel of changes in the patient’s condition appropriately.
  • Manages patients’ pain and symptoms pursuant to physician’s orders; obtains changes in orders as needed, and updates the patient’s care plan and medication list accordingly.
  • Responsible for the day-to-day plan of care management of assigned hospice patients, which includes:
    • Works with the patient, family, physician, and other care disciplines to develop and achieve care plan goals.
    • Case management of the hospice patient from admission to discharge.
    • Coordination of skilled nursing visit and services needed, including Patient Care Technician (PCT) assigned to the nurse’s patients.
    • Orders and documents durable medical equipment as needed.
    • Demonstrate excellent patient assessment skills.
    • Identify initial and ongoing patient and family needs.
    • Work collaboratively with Reid Hospice IDG to coordinate care needed.
    • Completes documentation for insurance reimbursement as needed.
  • Makes home visits to render professional nursing care including but not limited to:
    • Patient admissions, recertifications, and discharges from services.
    • Performs basic nursing care and comfort measures.
    • Anticipates potential complications and makes and makes appropriate changes.
    • Develops teaching plans and initiates teaching to patient, family, caregivers, volunteers, and other members of the health team as appropriate, and observes return demonstration.
    • Maintains flexibility in adapting to home environmental settings.
  • Responsible for phone triage calls to Reid Hospice, which includes:
    • Receive phone calls from hospice patients and families, assess needs of the call, and coordinate needed care over the phone, including scheduling home visits as needed.
    • Makes telephone assessment using sound nursing judgment and manages case by telephone intervention or home visit, as indicated.
  • Assists in evaluation of patients and families for admission into Reid Hospice, when assigned.
  • Demonstrates knowledge of various nursing, hospital, and home care policies/procedures, practices and standards for the purpose of clinical application, problem solving, and staff development.
  • Reports information accurately, concisely, and in timely manner.
  • Documentation of all services to be done within 24 hours of visit unless extenuating circumstances arises.
  • Is familiar with Medicare and Medicaid programs, and third party payer reimbursement.
  • Demonstrates at the end of the probationary period competency and proficiency in catheters, dressings, IV’s medications, and durable medical equipment.
  • In the event of an illness, vacation or other reason for absence, communicates with Hospice Charge Nurse to arrange patient visits to be completed by another RN Case Manager
  • Conducts PCT supervisory visits in accordance with Hospice policy, documents in the clinical record and communicates changes in patient condition and revision in care.
  • Participates in bi-weekly Hospice Interdisciplinary Group Team Meeting.
  • Maintains expected standards of quality and completes record review appropriately.
  • . Participates in inservices, department meetings, and assists with clinical experience for affiliate students and new employees.
  • May assist with volunteer training, long-term care facility inservices, and bereavement follow-up as assigned.
  • Works flexible hours as needed.
  • Rotates “on-call” schedule with other Reid Hospice nurses.
  • Other
    • All other duties as assigned including department-specific functions and responsibilities
    • Performs other duties as assigned and participates in organization projects as assigned.
  • Education/Experience

    Education Required: Must be a graduate of an accredited school of nursing.

    Education Preferred: Bachelor of Science in Nursing.

    Experience Required: Two or more years of clinical nursing experience.

    Experience Preferred: Experience in Critical Care or Medical/Surgical; must have a broad understanding of age specific needs.

    Licensure: Currently licensed as a Registered Nurse in good standing in the State of Indiana.

    Certifications: Must be Basic Life Support (BLS) Certified through American Heart Association.

    Other:

    • Must have appropriate background check completed for state of Indiana. Must not have been found guilty of: rape; criminal deviate conduct; exploitation of an endangered adult; failure to report battery; neglect or exploitation of an endangered adult; theft IC35-43-4 (within past 10 years) per IC 35-43-4.
    • Must hold a current driver’s license and have own transportation available at all times. A liability insurance verification must be signed and submitted to Reid Health Risk Management.
    Schedule Details
    Day Shift. 40 hours weekly. Monday - Friday 8-430p with a weekend and holiday rotation.
