Job summary

A leading health care provider is seeking a Hospice RN Case Manager to enjoy independence and flexibility while delivering compassionate in-home hospice care to patients across the Southern Twin Cities area. This full-time role offers an annual salary between $84,905 and $101,233 along with comprehensive benefits, including medical insurance, PTO, and more. Ideal candidates will have a valid RN license in Minnesota, a passion for patient care, and a minimum of one-year hospice experience.

Hospice RN Case Manager: Enjoy independence and flexibility in the field to visit patients one on one.

Hours: Full-time | Days | Monday - Friday with On-Call rotation.

Territory: The RN will have a general territory within our service area where they will provide in-home hospice care to the patients. The territory for this position would primarily be in the Southern section of the Twin Cities Metro area.

A Few Must-haves For This Position

  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of MN. License is active and in good standing.
  • Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate by the DHCS.
    • If a new graduate, 1200 verified hours of employment as a graduate or registered nurse within the last twelve (12) months or evidence of completion of a company-approved Preceptor Program.
  • One (1) year of hospice care experience preferred.
  • Able to travel within a geographic area serviced by the office. Reliable transportation, valid drivers license, and proof of auto insurance is required.
  • CPR Certification is required
