AMEDISYS HOME HEALTH in Milford, Delaware is seeking an RN Case Manager for a full-time day-shift role. The position covers a patient territory in Sussex County with orientation at the Milford care center, collaborating with the care team to plan and implement ongoing patient care.

Qualifications

• Minimum of 1 year of nursing experience.
• Current RN license for the assigned state.
• CPR certification and valid driver's license required.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other hospice care disciplines as needed or document rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records with accuracy and timeliness.
• Record pain/symptom management changes/outcomes as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Regularly communicate with clinical manager and care team.
• Perform on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
