Job summary

Amedisys is a leading home health and hospice provider in the United States. We are seeking an experienced RN Case Manager to support patients at home, coordinating care and ensuring optimal outcomes.

You will perform patient assessments, collaborate with the care team, and develop individualized plans of care. Travel to patient homes is required, with access to flexible scheduling and comprehensive benefits.

Qualifications

• One year of nursing experience required.
• Current RN license valid for the assigning state(s).
• Current CPR certification required.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Assess patients and help develop a care plan with the team.
• Refer to hospice disciplines when needed or document rationale.
• Maintain patient care progress records accurately and timely.
• Record pain and symptom changes and outcomes.
• Educate patients, families, and caregivers on care techniques.
• Communicate regularly with the clinical manager and team.
• Provide on-call coverage as assigned.
• Perform other duties as assigned.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
