Job summary

Harbor Healthcare System is seeking a qualified RN Case Manager for an outpatient position in the Independence area. The RN will coordinate admissions, assessments, discharges and pronouncements as part of an interdisciplinary care team.

The ideal candidate holds a Missouri RN license, with 1+ years of RN experience preferred, and is willing to travel. Excellent communication skills and a clean background are required. Working hours are standard with potential on-call and weekend rotation.

Qualifications

• Licensed RN in Missouri (required).
• Experience with Hospice or Home Health is preferred.
• Strong written and verbal communication skills.

We are currently seeking qualified candidates for a full time Registered Nurse (RN) Case Manager position in the Independence area.

Job Description

This is an out-patient position, that requires an independent and experienced RN. The RN-Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family. RN-Case Managermain job duties include and are not limited to;admissions, assessments, discharges and pronouncements.

Qualifications

• Licensed RN in the State of Missouri- required
• 1+years'experience as a RN - preferred
• Experience with Hospice or Home Health- preferred
• Effective written and verbal communication
• Ability to travel, must havereliable transportation
• Clean background,drug screen, and driving record
