Job summary

PACS is seeking a Case Manager – Registered Nurse to coordinate and deliver patient-centered end-of-life care. The RN will serve as the primary clinical contact for patients and families, ensuring care is timely and aligned with goals and values.

Responsibilities include assessing needs, developing care plans with the IDG, providing direct nursing care and symptom management, and educating families about plans, medications, and progression.

Qualifications

  • Active RN license in good standing.
  • Minimum 2 years nursing experience with hospice/geriatrics.
  • Experience providing direct care to patients with life-limiting illnesses.
  • Strong assessment, critical-thinking and clinical decision-making skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently while collaborating in an IDG.
  • Proficient with electronic medical records systems.
  • Valid driver’s license and reliable transportation for home visits.

General Purpose

The Case Manager – Registered Nurse will coordinate and deliver comprehensive, patient‑centered end‑of‑life care. The RN Case Manager serves as the primary clinical point of contact for patients and families, ensuring that care is individualized, timely, and aligned with the patient’s goals and values.

Essential Duties Patient Care & Case Management
  • Serve as the primary nurse responsible for coordinating all aspects of patient and caregiver care
  • Conduct comprehensive nursing assessments to identify physical, emotional, psychosocial, and spiritual needs
  • Develop, implement, evaluate, and update individualized plans of care in collaboration with the interdisciplinary group (IDG)
  • Provide direct nursing care, symptom management, and clinical interventions in accordance with hospice standards and protocols
  • Promote patient and caregiver independence whenever possible
  • Provide compassionate education and support to patients and families, ensuring understanding of care plans, medications, and disease progression
  • Maintain ongoing communication with physicians, team members, and caregivers to ensure continuity of care
  • Respond promptly to changes in patient condition and adjust care plans accordingly
  • Participate in on‑call rotation as assigned to ensure 24/7 patient support
Interdisciplinary Group (IDG) Collaboration
  • Attend and actively participate in IDG meetings and case conferences
  • Collaborate with social workers, chaplains, CNAs, volunteers, and other team members to address patient and family needs
  • Identify when additional services or disciplines are needed and initiate referrals appropriately
  • Maintain strong, respectful working relationships with all team members
Documentation & Compliance
  • Complete accurate, timely, and concise clinical documentation in accordance with organizational policies and regulatory requirements
  • Ensure all care delivered meets state, federal, and accreditation standards
  • Maintain thorough records of assessments, interventions, communications, and care plan updates
Professional Accountability
  • Maintain current RN licensure, CPR certification, and required continuing education
  • Demonstrate competency in hospice nursing practices, symptom management, and communication skills
  • Stay informed of current trends, best practices, and regulatory changes in hospice and palliative care
  • Represent the organization professionally with patients, families, healthcare partners, and community agencies
  • Exhibit strong time‑management skills and the ability to prioritize in a dynamic environment
  • Perform additional duties as assigned to support patient care and team operations
Qualifications (Education and/or Experience)
  • Current, active, and unrestricted RN license (compact/multistate license preferred)
  • Minimum 2 years of nursing experience, with at least 1 year in hospice, palliative care, home health, geriatrics, oncology, or related field
  • Experience providing direct care to patients with life‑limiting illnesses
  • Strong assessment, critical‑thinking, and clinical decision‑making skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently with minimal supervision while collaborating effectively within an interdisciplinary team
  • Computer proficiency and comfort with electronic documentation systems
  • Valid driver’s license and reliable transportation for home visits
Physical Demands
  • Ability to safely lift, move, and transfer patients using proper body mechanics; may require lifting or supporting up to 50 pounds
  • Frequent standing, walking, bending, kneeling, and reaching during patient care
  • Ability to assist with mobility, repositioning, and comfort measures in varied home environments
  • Stamina to sustain prolonged physical activity and respond effectively during urgent situations
  • Ability to drive between patient homes or facilities as required
  • Adequate visual, auditory, and tactile acuity to assess patient conditions and perform clinical interventions safely
  • Ability to operate medical equipment and manage supplies in home‑based care settings
