Job summary

Amedisys is seeking a Registered Nurse case manager in Georgia to support home health patients during day shifts. You will assess patients, coordinate with the care team, and implement individualized plans of care in collaboration with physicians and family members.

The role emphasizes on-home visits, on-call availability, and maintaining detailed progress records, while providing compassionate support to patients during illness and recovery.

Qualifications

• Minimum 1 year of nursing experience.
• Current RN license valid for the state(s) you work in.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop a care plan.
• Makes referrals to other hospice disciplines as needed and documents rationale when not referring.
• Maintains individualized patient care progress records accurately and timely.
• Records pain/symptom management changes and outcomes.
• Educates patients, families, and caregivers on care techniques.
• Communicates regularly with the clinical manager and care team.
• Performs on-call responsibilities as assigned.

• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
