Job summary

St. Croix Hospice in Michigan is seeking a Registered Nurse Case Manager to provide 1:1, whole-person care to patients in their homes, coordinating with an integrated care team to create individualized plans.

You will educate families and support them through complex decisions with professionalism and empathy. You will educate patients and caregivers on care techniques, medications, diet, and symptom management, and rotate on-call duties to ensure seamless transitions and high-quality home

Qualifications

• RN with 1+ years of acute care experience; hospice/home health preferred.
• Complete comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patient and family.
• Educate patients and caregivers on care techniques and meds.
• Rotate on-call duties and ensure seamless care transitions.
• Must have reliable transportation, valid driver’s license, and proof of auto insurance.

Responsibilities
