Residential Home Health and Hospice in Michigan is seeking an RN Case Manager to lead and collaborate on patient care plans across Tecumseh, Adrian, Britton, Macon and surrounding areas. Eligible for a $10,000 first-year bonus based on achieving key goals during the first year.

You will work in a team-based home health setting, visiting patients, charting diligently, and using tablets and EMR tools to support care delivery and coordination across multiple locations.

Qualifications

• RN license in the state you work.
• Graduate from an approved school of practical nursing.
• One year of RN experience and the clinical competence to deliver quality patient care.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• Commitment to consistently meet critical deadlines for charting.
• Skills to self‑manage time and schedule.
• Experience with tablets, mobile phones and EMR software.
