Optum is seeking a Nurse Case Manager to guide patients and families through the care continuum, coordinate care, support discharge planning, and manage outpatient care under supervision of an RN or MD.

We offer a $7,500 sign-on bonus and a comprehensive benefits package as part of the Optum family. The role includes collaboration with Care Advocates and Social Workers to ensure appropriate levels of care and ongoing patient education.

Formación

• Current RN license, specific to assigned state(s).
• 1+ years of clinical experience as an RN.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsabilidades

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health through teaching and rehabilitative measures; supports self-care.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Participates in on-call responsibilities and services.
