Job Summary:

Provides healthcare integration for clients from pre-admission to post-discharge through coordination and sequencing of care, promoting early intervention and interdisciplinary communication. Uses independent nursing judgment and age-specific care methodologies across various patient populations, including neonatal to geriatric. Communicates effectively with patients, families, and team members. Monitors and evaluates patient responses, documents clearly, and demonstrates critical thinking, independence, and organizational skills. Works primarily day hours, with scheduled weekends and occasional evenings. May be exposed to communicable diseases and hazardous materials.

Qualifications:
  • Bachelor’s degree in nursing preferred
  • Graduate from a nationally accredited nursing program preferred
  • One to two years of professional nursing experience preferred
  • Unencumbered RN license with the State Board of Nursing or multistate licensure
  • Department-specific competencies and certifications required
  • Valid driver’s license may be required depending on location
