Job summary

CarePartners Health Services is seeking a Primary Registered Nurse Case Manager for Home Health in western North Carolina. You will provide direct patient care per physician orders, follow the plan of care, and work closely with the care team to achieve positive patient outcomes.

Responsibilities include coordinating hospice services, communicating needs to the interdisciplinary team, and managing pain and symptom control to ensure comfort and quality of life for patients and families.

Qualifications

• Basic Cardiac Life Support (BLS) certification required within 30 days of start date.
• Nursing license (RN) with Class C license.
• RN with Associate Degree in Nursing.

Responsibilities

• Provides direct patient care per physician orders and plan of care.
• Collaborates with interdisciplinary team to achieve positive patient outcomes.
• Coordinates hospice services and coordinates with Hospice Aide and Director of Clinical Services.
• Manages pain and symptom control for patients.

Job Summary and Qualifications

Responsible for providing direct patient care in accordance with physician orders, recognizing standards of practice and establishing company policies and procedures. Responsible for following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

What you will do in this role:

• Follows the plan of care, provides treatments, and works collaboratively with the members of the team to help meet positive patient care outcomes
• Provides direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care
• Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group, including Hospice Aide, Director of Clinical Services –Hospice, and Chaplain
• Provides appropriate pain/symptom management

What qualifications you will need:

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• Class C License
• Registered Nurse
