Job summary

Qualifications

• One (1+) year of clinical RN experience; VP approval if under 1 year.
• Current RN license for assigned state.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Assess patients and collaborate with care team to plan care.
• Make referrals and document rationale for care decisions.
• Promote health and independence through teaching and rehab measures.
• Supervise LPNs and HHAs.
• Document timely and accurately.
• Communicate patient progress to clinical manager.
• Plan staff education and participate in on-call duties.
• Engage in ongoing clinical development and education.
