United Energy Workers Healthcare is seeking a Registered Nurse Case Manager to assess patient needs, coordinate care, and communicate with the care team. The role involves home visits and travel to support energy workers and their families with comprehensive, patient-centered care.

The ideal candidate holds an active RN license with 1+ year home health experience, strong assessment and organizational skills, tech savviness, and the ability to pass background checks.

Qualifications

• Current RN License: Valid and active RN license in good standing.
• Experience: At least 1 year in home health or related setting with strong assessment skills.
• Organizational Skills: Excellent organization and time management.
• Tech Savvy: Proficient with computers and internet-based applications.
• Equipment Knowledge: Competent with client-based equipment.
• Background Check: Must pass criminal background check and sanctions screening.
• Professional Appearance: Professional demeanor and appearance required.

Responsibilities

• Manage Nursing Care: Coordinate nursing care, home health aide services, and related therapies as approved.
• Ensure Confidentiality: Maintain HIPAA and Privacy Act PHI compliance.
• Facilitate Communication: Ensure effective collaboration among the care team.
• Additional Duties: Perform other duties to support the team and patient care.
