Job summary

Ultimate Home Health Care LLC, a Pillar Health Group Company, is seeking Registered Nurse Case Managers to deliver high-quality home healthcare across Dallas-Fort Worth and surrounding areas. Full-time opportunities with flexible schedules and comprehensive benefits are available.

Responsibilities include in-home care, documenting with OASIS/Kinnser, and responding to PRN emergencies. Texas RN license, driver B license, and 1+ year home health experience are required.

Qualifications

Graduate of an accredited Nursing Program.
Active Texas RN License and Driver's License.
Minimum 1 year of Home Health experience.
Experience with OASIS/Kinnser preferred.
Ability to work flexible schedules and travel locally.
License/Certification:RN License (Required).
License/Certification:Driver's License (Required).
Registered nurse Texas (Required).
Willingness to travel:100% (Required).
Experience:Home Health:1 year (Required).

Responsibilities

Provide professional, compassionate care in clients' homes.
Manage diverse client needs with Home Health expertise.
Document care accurately using OASIS/Kinnser.
Respond to PRN emergency calls as needed.
