Job summary

Amedisys Home Health in Lancaster, PA is seeking a Registered Nurse Case Manager to deliver patient-centered home health services. This full-time, dayshift role coordinates care, completes assessments, and guides patients and families to achieve safe, independent healing at home.

You will supervise LPNs and HHAs, document timely, and communicate patient progress to the clinical team. Requires 1+ year RN experience, active RN license in the assigned state, CPR certification, a valid driver's

Qualifications

• 1+ year of RN clinical experience.
• Current RN license for assigned state(s).
• Current CPR certification.
• Valid driver’s license and liability insurance.
• Note: RN Intern program may be required if <6 months RN experience.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs and documents rationale.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes timely and accurate documentation.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and services as assigned.
• Participates in clinical development and continuing education.
